Population Health Management

SBS's Population Health Management (PHM) vendors work with employer, health plans, and other health providers to identify, control, and manage the health risks of their employee or member populations.

We contract with various vendors that go well beyond the many traditional utilization and disease management programs in the marketplace. We offer a fully integrated, completely customizable health management solution that motivates people to take control of their health at every level, whether they are the model of health or have a chronic condition.

• For individuals who are at high risk for future problems: PHM will help identify risks in advance, in time to take action, then proactively motivate and support those individuals with a customized program that will help to (1) effectively ater their health behaviors, and (2) significantly change their risk profile—as well as their health outlook.
• For individuals with a chronic condition or an existing illness: PHM will manage and control their condition and improve their quality of life.
• For healthy individuals: PHM will help them stay that way.
• SBS can provide electronic data files to PHM vendors to streamline the process and assure members are identified for services in a timely manner.
